Worth Haven for Veteran Program Application

Before you apply, please ensure that you meet all eligibility criteria for our program application. Each applicant must fulfill specific requirements to qualify, ensuring a smooth and efficient process for everyone involved. Review the criteria carefully to determine your eligibility and prepare to submit your application.
 
Applicant must meet the following criteria to be considered for admission:​

  • Must be a resident of North Carolina

  • Must be a Veteran aged 21-55

  • Must have served in the U.S. Armed Forces

  • Must have low to moderate income

  • Must be able to meet payment requirements

  • Must not have a prior conviction for arson or sexual crime

  • Must not have schizophrenic or pica disorder

  • Must be medically, physically, and mentally stable

  • Must be stable on medication, medically compliant, and have case management

  •  The following medications: Lexapro, Celexa, Paxil, Xanax, Valium, Ativan, and Klonopin are not allowed unless monitored by a doctor. If not, a higher level of care is required than what we can provide.

  • If diagnosed with post-traumatic stress disorder, an individual must have treatment longer than 6 weeks

  • Must attend an intake interview with the Foundation's Staff

  • Must have at least 6 weeks of treatment if diagnosed with PTSD

  • Must lack suitable housing

  • Must have renter's insurance

  • Must be committed to personal growth and development

  • Must be willing to participate in NA/ AA/ EA, personal development, and life-skills programs
